This Tuesday our team was back in Brisbane Carina Clinic for another busy round of vasectomies. It was great meeting so many men, some of whom had been putting off getting the snip for years!

One question I get asked almost every day is why men need to wait three months and have at least 30 ejaculations before getting their sperm test done.

After all, if we've cut and sealed the tubes, shouldn't the job be done?

Well, not quite.

It's Like Cleaning a Coffee Machine

During a vasectomy, we snip and seal the two tubes (vas deferens) that carry sperm from the testicles. We also place a layer of tissue from the spermatic cord between the cut ends to help prevent them from ever reconnecting. This is called fascial interposition and provides an extra safeguard against vasectomy failure.

Each tube is around 30–45 cm long, and although we've stopped new sperm getting through, there's still sperm left in the tubing beyond where we've made the cut.

Even after you've stopped the coffee going in, there's still some left in the pipes that needs flushing out.

I often compare it to cleaning a coffee machine. Even after you've stopped the coffee going in, there's still some left in the pipes that needs flushing out.

When a man ejaculates, muscles around these tubes contract, helping push the remaining sperm out with the semen. That's why I recommend at least 30 ejaculations before testing.

And no, we certainly do not recommend trying to complete those 30 ejaculations in the first week!

So Why 3 Months?

Because every man is different. Some clear their remaining sperm quickly, while others take longer. Waiting three months gives the body enough time to clear most of what's left behind.

At that point, we ask for a semen sample to check whether any sperm remain. Occasionally, another test is needed, but this doesn't necessarily mean the vasectomy has failed.

Most importantly, keep using contraception until we've reviewed your results and given you the all-clear.

So Brisbane, enjoy the next 3 months, get those 30 ejaculations in, and don't forget to send us your sperm test!

Until next month,
